Direct Support Professional

Overall Purpose of Job: To support persons served by the agency in living the fullest and richest life that they choose.

Main activities and tasks:

Advocacy:

  • Educate and inform individuals as to the resources around them
  • Encourage individuals to share their wants, needs and desires
  • Create opportunities for their voices to be heard and choices to be made
  • Willingness to accept direction from the individuals who receive service
  • Outcome Achievement:

  • Implementing support strategies designed to assist the achievement of personal outcomes
  • Completing the necessary quality documentation of the process, progress and achievement of personal outcomes which can include daily, periodic of annual reporting
  • Participates in the planning process for the individual outcome achievement, which may include attendance at special meetings outside of scheduled work hours
  • Community Access and Involvement:

  • Facilitates natural relationships with others in their community (i.e. shopkeepers, bank tellers, church members, neighbors, etc.)
  • Assists individuals in exploring activities in their communities
  • Provides transportation as necessary in agency owned or personal vehicles
  • Health and well-being:

  • Is proactive regarding matters of health and safety
  • Assures that any known health or safety matter affecting the individuals is responded to in a timely and responsible fashion
  • Assists as needed in attending medical appointments, giving medications, and other delegated tasks
  • Completes all necessary documentation regarding medical, behavioral, dietary, therapeutic supports
  • Assures that personal care needs are met
  • Home Responsibilities:

  • Assists or completes household tasks as needed while encouraging individual independence (cooking, cleaning, planning, shopping, etc.)
  • Finance:

  • Maintains necessary documentation of personal and household purchases
  • Education, Qualifications and/or Special Training:

  • High School Diploma or equivalent
  • Able to obtain CPR, First Aid and State Medication Administration Certificate
  • Commitment to continuous enhancement of support skills
  • Knowledge and Skills:

  • Flexibility
  • Ability to make independent decisions and accept supervision
  • A commitment to treating people with dignity and respect
  • Willingness to be a team player
  • Ability to listen and communicate with others
  • Common sense
  • Special Requirements:

  • Over 18 years of age
  • Valid Connecticut Driver’s License. May need personal, insured vehicle for transportation of others
  • Proof of employment eligibility
  • Physical Requirements:

  • May have to lift more than 25lbs. and up to 75lbs.
